We Need to Verify Former Military Service on One of Our Applicants

The National Personnel Records Center (NPRC) maintains records for former military service members. To obtain information from NPRC, Standard Form 180, "Request Pertaining to Military Records," is required to be completed by the military service member. The fully executed form should then be sent to Cisive for processing. Please be advised that there is a significant delay in receiving a response from NPRC subsequent to their receipt of the completed SF 180.
